Determine Your Health Class

Your age, current health, and past medical history are all evaluated as part of your application for life insurance. As you apply for life insurance, you will be asked to determine your health class. Most insurance companies use general insurance rate guidelines to determine your health class.

The health classifications are:

• Best class/Preferred Plus

• Preferred

• Standard Plus

• Standard

Your underwriting class will be based on your current health, medical history, and additional underwriting information. Depending on the nature, severity, and treatment of your impairment, higher class rates may apply.*

To determine your health class, refer to the tables below that apply to you:

• For NON-Tobacco and NON-Nicotine Users
  Refer to this table if you have not used cigarettes, pipes, chewing tobacco, snuff, nicotine patches or nicotine gum in the last 12 months.
